Whether the person is being supported in a residential service, family support service or outreach service, each person will have in place one or a combination of the following plans;

Individual Educational Plan 

Trinity Support & Care Services recognise that interventions are more effective when the following is in place; Individualised goals and plans are developed that are based on specific evaluation/assessment undertaken with the child. For example: The young person or adult will take turns when engaging in desk top game. The selected goals should aim at improving interactions with peers. Making use of ‘natural environments’ if possible, rather than constructing “special‟ settings. Then the young person or adult is more likely to use the new skills in other settings as the supports and cues for these will be present.
